In order to participate in any school sports, the student must have completed the following forms:

• Season Information Sheet
• WVSSAC Student Physical form dated on or after May 1st
• Notarized Proof of Insurance Form
• Baseline Concussion Testing (Online)
• Signed Drug Testing Consent Form
